The Office of the Registrar General registers all births and deaths that take place in Ontario.
To apply for a birth certificate, visit the Service Ontario website.
If your baby is a newborn, you may use Service Ontario’s Newborn Registration Service to submit the Statement of Live Birth and to apply for a birth certificate, a social insurance number and Canada Child Benefits.
If your birth certificate is lost, stolen or damaged, you must report it immediately to the Office of the Register General.
